Access to Health Care Records

Clients have the right to request copies of their health care records.

To request your records, please submit a written request by email or through the client portal. Requests will be processed in accordance with Texas law and HIPAA regulations. If you have questions about this process, please contact the practice directly.

Filing a Consumer Complaint
